STEPS TO OBTAINING WORKING PAPERS
 
 
2. Have your prospective employer complete and sign Section C.
 
3. Have a physician complete and sign Section D.
 
4 Bring the application back to any middle school or the high school.
 
     ONE OF YOUR PARENTS MUST BE WITH YOU
 
     Have one of the following pieces of identification with you:
           Birth certificate or hospital certificate
           Baptismal certificate
         Driver"s license or permit
           Passport
 
5. Parent must be present to sign Section B in the presence of a designated School District witness.
 
6. Student must also be present to sign Working Papers in the presence of a designated School District witness and Working Papers will be issued.
